CCLAC Incubator - Agreement of Participation in 2023 Cohort Digital Literacy Program

I. Purpose of Agreement

The purpose of this agreement is to describe the terms, conditions, and rules regarding the participation of:

II. Minimum Qualifications for Participation

The Participant certifies that he/she/? is a United States citizen, a U.S. National, or a Legal Permanent Resident Alien. The Participant must be enrolled in high school, be working towards a GED, or be willing to agree to prepare for the GED test within one year of the enrollment in this Program with the understanding that the Participation Award Stipend given for participation in this Program is not available until they receive their GED or they graduate from high school. All Participants must be between the ages of 14 to 24 and all Participants under 17 years of age must have parental consent to be involved in the 2023 Cohort Digital Literacy Program.

III. Service Term (2023 COHORT) Duration and Rules

You are required to indicate your agreement by checking the boxes below.

A) The Participant's Term of Service begins on Tuesday Sept 5, 2023 and ends on Thursday November 16, 2023.

B) This Term of Service may be extended by the Participants and Organizations, in writing, if the Participants’s service enrollment has been suspended for a short period due to compelling personal circumstances.

C) The US-based Participants will log a minimum of 5 hours per week to complete the requirement of 50 hours of service during this period of enrollment in the Program. COBACH Participants will serve a minimum of 16 hours a week to complete the required 160 hours of service during this enrollment period.

D) Of these hours logged, a maximum of 20% of these hours will be used for:

1) learning how incubator.org works *following the USER GUIDE

2) Taking the applicable course on incubator.org to earn a Certificate of Completion

3) other similar approved activities.

An additional 10% of these hours may be used for fundraising activities.

E) The Participant understands that in order to be eligible for serving a second term of service, the Participant must receive satisfactory reviews of service for any previous term of service. The Participant also understands that eligibility for a second term of service will be based on at least a mid-term and an end-of-term evaluation of the Participant's performance focusing on the following 3 factors:

1) completed the required number of hours

2) satisfactorily completed assignments, tasks, or projects

3) met any other criteria that were clearly communicated both orally and in writing at the beginning of the term of service, during mid-term or end of term evaluation.
